Patient Participation Group at Cunliffe Medical Centre
Doctor Shackleton and Doctor Dickinson are inviting patients to join our 'digital' electronic Patient Participation Group. The group will be asked to complete a series of surveys on Practice services during the course of a year ( not too many!!) and will also be asked for opinions on changes proposed in the way the Practice works from time to time.
If you want to take part, please download the form here , and send back to us via e-mail or drop in at reception or post to us. All information we ask for is fully protected under NHS Information Governance and Confidentiality rules, and is in accordance with the Data Protection Act 1998 (as amended).
You must be a registered patient of Doctor Shackleton or Doctor Dickinson to take part. We need a broad cross-section of our patients in the group, young people, senior citizens, different ethnic groups, men and women, different medical conditions.
As an incentive to take part, patients who join the group and respond to our questionnaires will earn £1 per response for Derian House Childrens' Hospice in Astley Village donated by the practice.
